Transition Assessments

Recommended by TMCSEA Youth Services Department

Before writing the transition plan, assessments should be given to gather information to inform this section of the IEP.  There are two different categories for assessments formal and informal.  Formal assessments have been researched based and finalized by specific companies that support transition to adulthood.  Infomal assessments have been created to gain more information from the student/family.  These are typically used after the formal assessments.
